Note: The opt-out form is not required; please submit it only if you want Pickerington Schools to omit or exclude your student. Otherwise, your permission is assumed. Example: Opt-out of your child photo/video being taken.

Administering Nonprescription Medications
This form is to request the administration of non-prescription or over-the-counter (OTC) medications to your child.
Authorization for the Possession and Use of an Epinephrine Autoinjector
This form authorizes the possession and use of an epinephrine autoinjector by a student.
Food Allergy and Anaphylaxis Emergency Care Plan
FARE's Food Allergy & Anaphylaxis Emergency Care Plan, formerly the Food Allergy Action Plan, outlines recommended treatment in case of an allergic reaction, is signed by a physician and includes emergency contact information.
Immunization Requirements for Students Entering Grade 12
This form documents that a student entering grade 12 has received the recommended immunizations.

Ohio Legal Immunization Exemption
This form is to notify Pickerington Schools that a student is not in compliance with immunization recommendations. This form must be completely filled out and submitted with supporting documentation.
Physical Exam Form
For new students enrolling into kindergarten or grade 1. Please have your child’s health care provider complete this form and return it to your child’s assigned school building. Your child will not be cleared to attend school until this form has been signed and returned to the school on or before August 1.

Request for Administration of Prescription Medication by School Personnel Grades K-12
This form is to request the administration of prescription medication. It must be signed by your child's health care provider.
Request for Specialized Health Care Services - Parent Authorization
The Parent Authorization Form must accompany the Physician Authorization form.
Request for Specialized Health Care Services - Physician Authorization
Physician Authorization must accompany Parent Authorization.

Work Permits

Application for Minor Work Permit
This application should be completed by the student, the student's legal guardian and the student's prospective employer. The completed application should be submitted to the school in which the student is enrolled. If the student is enrolled in an alternative education setting (home schooled, GED) this application should be returned to the school district in which the student resides.

Physician's Certificate for Minor Work Permit
This form should be completed by a physician before a minor applies for a minor work permit.

If the student has previously submitted a physical for an organized school activity within the school year, this form is not necessary.

Minor Wage Agreement
Ohio Revised Code, Seciton 4109.10 requires a written agreement to wages between a minor and their employer.

The following form is furnished as an example, and may be reproduced by any employer. This form should be prepared in duplicate and signed by both the employer and the minor. One copy to be given to the minor and the other copy to be retained by the employer in the personnel file of the minor.

Parent or Guardian Consent form
Minors aged sixteen or seventeen who are to be employed during summer vacation months after the last day of the school term in the spring and before the first day of the school term in the fall, in nonagricultural and nonhazardous employment as defined by the ''Fair Labor Standards Act of 1938'', 52 Stat. 1060.29 U.S.C.A. 201, and similar state statutes, or in other employment not prohibited to minors age sixteen or seventeen by law, shall not be required to
provide an age and schooling certificate as a condition of employment. In order to be hired for employment during summer vacation months, such minors shall provide the employer with the following:
1. Evidence of proof of age in the same manner as proof of age is provided the superintendent of schools under division C of section 3331.02 of the Revised Code.
2. A statement signed by the minor's parent or guardian consenting to the proposed employment during the summer vacation months. For the purpose of this section, in the absence of a parent or guardian a person over eighteen years of age with whom the minor resides may sign such statement.
The employer shall retain a copy of the proof of age and the statement of consent with the minors employment records.

Beginning with the 2024–2025 school year and continuing thereafter, Pickerington Local Schools will no longer offer Intra-District Transfer options for K–12 students. This change reflects alignment with Board Policy 5113.011 AG and applies to all current and future academic years.
